This report was sent anonymously by a whistleblower at Bristol children's hospital after Luke's inquest. It criticises the previous serious incident report that was given to the coroner

Please note the second paragraph in the executive summary 'The Panel did not have the clinical notes or the Child Death Review (CDR) meeting minutes which led the Panel to be unable to support and/or comment on the root causes as identified in the RCA and 1-3 of the panel's terms of reference'.


This is in reference to the serious review panel who tried to discredit the root cause analysis , and also when Mr Woolley was questioned as to why the serious review was carried out he gave a response that it was to look into the investigation processes that took place for Luke's death and not to explain why he died or how he died. As you will see in the serious incident review report the panel of four divisional managers clearly comment on whether Luke's death was preventable and as the leaked report shows the serious panel did not even look at Luke's medical notes, nursing notes or observation charts to come to any conclusion about Luke's death!
